Transcript requests can be submitted via email or by contacting Mrs. Charvat in the counseling office. Please send an email to lcharvat@smokyvalley.org with the following information:

Name (current and maiden if applicable)
Year of Graduation
Where the transcript needs to be sent (both the name of the institution and the address if possible)

Transcripts will be processed within 48 business hours of the request.
